You can get an application in the following ways:
1) In Person: pick up a copy at our office at 2012 Pine Street, 2nd Floor, San Francisco, 94115.
2) Download: you can get the application here.
3) Apply Online: access and complete the online application using this link, and download the Required Documents Packet here.
Be sure to turn in all completed applications and Required Documents Packets (if applying online) by Friday May 7 at 5pm.
Applications must be RECEIVED in our office (not postmarked) by deadline to be considered. Online applicants MUST complete a Required Documents Packet and submit by deadline.
